①「 抑うつと無力感:戦術と技術」

このワークショップは、抑うつと、特に無力感のテーマを扱う。まず最初に、抑うつについて概説し、それから無力感という概念につなげる。治療では、個々の患者の変化への準備段階を支持する必要があることに注意を喚起したい。変化への準備段階の輪郭を描き、これは患者が変化への準備ができることを狙った特定の治療的戦術・技術を、決定する道筋として用いられる。これらの戦術と技術は、行動学的、認知行動学的パラダイムに負うところがかなりあるが、これらのパラダイムが精神力動的なねらい・目的をいかに補うかについても説明する。プログラムの全体的な目標は、さもなければ患者の抵抗とみなされてしまうような事象への対応に役に立つ戦術と技術のいくつかを参加者に提供することにある。また、患者へのより全体的なアプローチを提供することを目指しており、それは通常用いられているよりもずっと協力的で教義的なやり方になる。最後に、統合された行動学的健康プログラムを紹介する。これは、メンタルヘルスの機関で普通は出会わない心理学的な問題を持つ身体疾患の患者への対応として有効である。

② 「心理力動的外来集団精神療法:PTSDとうつ力動からの解放」

 IADPが向き合い続けている東日本大震災後の大規模PTSDや「うつ」からの解放には、力動的集団精神療法が欠かせない。また、我々の専門とする青年期困難患者が頻繁にあらわす自殺未遂、自傷行為等の自己破壊的行動化の奥にも、複雑性/累積PTSDやうつが存在する。本ワークショップは、ロールプレイなどの体験演習や小講義を用いて、1)見えにくいPTSDや、うつ力動を可視化し、的確な査定(トリアージ/セラグノーシス)を行うための基礎理論を学ぶこと、2)その治癒を促進する力動的集団精神療法の実践イメージと基本技法に習熟することを目的とする。力動の世界に初めて触れる初学者の方から、ベテラン臨床家の方まで、心の重荷を解放し、腹の底から元気になる力動的集団精神療法の世界への第一歩を、熊本から始めてみましょう。 

③ 「『僕の古い友達、暗闇君、こんにちは』―青年のうつに取り組む」
“Hello Darkness, My Old Friend”-Working with Adolescent Depression

 青年期のうつは、その絶望の中にある青年の窮状を捉え、喚起させられるかのように様作家や詩人によって描かれてきた。シェークスピアのハムレット、サリンジャーのホールデン・コールフィールド、シルビア・プラスの『ベル・ジャー』は、みなうつの青年のありさまである。精神分析学の文献、フロイト、アブラハム、そして後には、スピッツ、ボウルビー、アンソニーは、診断と治療法についての理解に貢献している。またよく知られているようにアーロン・ベックと彼に続く人々によって開発された認知療法もまた、治療戦略の発展において非常に貴重である。このワークショップでは、青年期のうつの広がり、この年齢層に「当たり前に起きる」うつと精神病理としてのうつを何によって識別するのかを探る。参加者は、診断について学ぶ。また、個人療法、集団精神療法、家族療法を用いたときの、多様な治療的反応についても検討する。

④ 「対人援助職のための応答構成入門」
  トレーナー:能 幸夫(PAS心理教育研究所)

 力動的視点を据えた精神医学、精神看護学、心理療法、ソーシャルワークは、人と人が心理的にコンタクトを取り、相互作用が生じる場における対話や応答がそのまま治療的あるいは援助的な介入になっていきます。わたしたちは、どのように生きた対話や意味ある応答を創りだしているのでしょうか。
 応答構成訓練は、具体的には、時間をとめて、ある場面における患者さんやクライエントの言葉を前にして、自分の応答を構成していくプロセスを吟味し検討していく訓練です。  今回の入門講座の対象は、医師、看護師、臨床心理士、ソーシャルワーカーといった対人援助の専門職の方々です。
 ともに楽しくそして有意義な訓練にしていきましょう。

⑤ 「プレ・セラピィ技法:SMGとSET」

 助けの必要がありながら、相談に来られない、相談への動機がもてない人々に対して、心理療法への準備態勢を整える処方がプレセラピィである。我々は2011年、東日本大震災以降、引きこもり、助けを拒絶する人々に多く直面する中で本処方を展開してきている。プレセラピィは、心の機能を活性化し、精神内的な安全感を確かなものにすることで、自分を育て、鍛えることに抵抗する力や無力感を緩和する。本訓練では、Story Making Group(SMG)、Socio-Energetic Training(SET)2つのグループ処方を紹介する。参加者は実際に2処方を体験し、現代におけるプレセラピィ処方の必要性と、2つの各技法の臨床的意義、そして本処方における基礎技法を学ぶことが目的である。

⑥ 「力動的精神看護介入技法、-セルフケアの促進のための力動的看護面接-」

 精神看護において、オレムーアンダーウッドのセルフケアモデルは、人間関係の看護論を展開したヒルデガード・ペプローに続き、日本では普遍的に用いられるケアモデルとなってきています。そしてこのケアモデルは、脆弱性ーストレスー対処モデルと共に、患者の理解のために精神力動理論が用いられていますが、これについては正確に理解されていません。また、オレムーアンダーウッドモデルでの患者のセルフケア促進のための看護過程の展開においては、力動的看護面接が変化の要となっているにも関わらず、その方法論については個々人の看護師が自分の考えで展開しているにとどまっています。そこで、今回、オレムーアンダーウッドモデルを正しく理解、活用するために、①精神力動理論とオレムーアンダーウッドモデル、②セルフケア促進のための力動的看護面接の技法、③患者・家族・看護師集団の凝集性と機能を高めるための看護者の介入技法について、講義、事例検討、ロールプレイを通じて深めていきます。

⑦「組織開発の力動と介入法」

 組織開発の理論と実践は、行動科学だけでなく、精神分析および集団精神分析の理論によって大きく発展を遂げてきている。エグゼクティブコーチングもまた、その恩恵を受けてきている。私のアプローチは、力動的心理療法、集団精神分析、そして一般システムズ理論から再構築、統合したものである。  このプログラムは、心理療法の仕事が効果的にできるように、医療専門家や対人援助従事者の組織に焦点を合わせている。特に陰性治療反応(NTR)を伴う困難患者の力動的心理療法は、それが行われる機関を様々に揺るがす。機関の環境全体が心理療法の仕事を揺るがし、きわめて破壊的な影響をもたらすこともある。どのような組織で働く心理療法家も、安全に自分の心理療法バウンダリーを守るために、リエゾンワークを行う必要がある。心理療法を効果的に行うには、その組織の中に、充分に心理学的心性に馴染んだ環境が必要である。力動的心理療法家としては、心理療法によって患者の治癒を助けるとともに、心理療法にとって最適の環境を組織内に開発していかなければならない。 本プログラムでは、個人と組織の相互作用ダイナミクスの基礎理論と、それを生産的かつ創造的に使えるようになるための分析と介入の手法を学ぶ。
 *各参加者は、ご自分が心理療法もしくは関連の仕事をしている所属機関で組織展開上にぶつかる問題を事例報告メモの形で準備してご持参下さい。

1. Depression & Helplessness: Tactics & Techniques

Trainer:Ralph Mora, Ph.D., MSS, CAIA (Reynolds Army Community Hospital)

This workshop encompasses the topic of depression and in particular, helplessness. Initially, an overview of depression is presented leading to the concept of helplessness. Treatment is noted to require adherence to the individual patient’s stage of readiness for change. These are then outlined and used as a method of deciding on specific therapeutic tactics and techniques meant to allow the patient to become ready for change. These tactics and techniques borrow greatly from behavioral and cognitive behavioral paradigms but are also explained with regard to how these complement psychodynamic aims and objectives. The overall goal is to present to the participant a number of useful tactics and techniques meant to address what is otherwise seen as resistance from patients. These are also meant to provide a more holistic way of dealing with patients that is more collaborative and didactic than is generally used. Finally, an integrated behavioral health program will be presented as a way of addressing medical patients with psychological problems that are usually never seen in mental health centers.

 

2. Out-Patient Psychodynamic Group Psychotherapy:Liberation from PTSD and Dynamics of Depression”

Psychodynamic psychotherapy is essential for liberating from the major PTSD and “depression” after the Great East Japan Earthquake, which IADP has been facing. We can also find complex/accumulated PTSD or depression in the inner core of the adolescent difficult patients, our major clients, who often show acting out including a suicide attempt or self-destructive behavior. This workshop will aim to 1) learn the basic theory of accurate assessment (triage/theragnosis) by visualizing less-visible dynamics of PTSD and depression, 2) train basic techniques and practical images of dynamic group psychotherapy to promote healing. For anyone from beginner who encounters the dynamics approach for the first time to professional clinical practitioner, let’s take the first step from Kumamoto to the world of dynamic group psychotherapy which can liberate people from their burden and make them cheerful.

 

3.‘Hello Darkness, My Old Friend”-Working with Adolescent Depression

Depression in adolescents has been described by authors, and poets in ways that are evocative and capture the plight of the adolescent in the throes of despair. Shakespeare’s Hamlet, Salinger’s Holden Caulfield and later Sylvia Plath’s The Bell Jar all offer examples of depressed youth. In the psychoanalytic literature, Freud, Abraham and later Spitz, Bowlby and Anthony contributed to our understanding of diagnosis and treatment. The cognitive therapies, notably those developed by Aaron Beck and his followers, are also invaluable in helping to develop treatment strategies. In this workshop, we will explore the prevalence of depression in adolescence, and what distinguishes ‘normative’ depression in this age group from psychopathology. Participants will learn about diagnosis. We will also explore various therapeutic responses through the use of individual, group and family therapies.

 

4. Introductory of Constructed Response Method for Interpersonal Responders

In psychiatry, psychiatric nursing, psychotherapy and social work with psychodynamic view point, dialog or response in person to person contact or in the interaction can become therapeutic or helpful intervention. How do we create lively conversation or meaningful response? Response-Constructing training reviews and discusses the process of your response to the client by stopping the time and facing the verbatim of the client in a moment of the case. This introductory program is intended for interpersonal aid professionals including doctors, nurses, clinical psychologists, and social workers. Let’s enjoy training and make this program fruitful.

5. Technique of Pre-Therapy:SMG and SET

Pre-Therapy is a prescription for those who cannot come to consult or has no motivation for consulting even though they need help, to accommodate their readiness for psychotherapy. We have met many people who withdrew and refused help after the Great East Japan Earthquake of 2011 and developed this technique. Pre-therapy relieves the power resisting self-nurturing and self-training or helplessness by energizing psychological function and securing their inner security. This training program will introduce two types of group therapy: Story Making Group (SMG), and Socio-Energetic Training (SET). Participants will experience these two types of technique and learn the necessity, clinical meaning, and the basic technique of these two techniques.

6. Intervening technique of Psychodynamic Psychiatric Nursing –Psychodynamic Nurse Interview to Promote Self Care –

Orem/Underwood self-care model in psychiatric nursing is universally used in Japan following the model of Hildegard Peplau who developed interpersonal relations in nursing. This self-care model use psychodynamic theory to understand patients as well as Diathesis-stress coping model but this is not fully understood yet. In addition though psychodynamic nursing interview is the key for the change of patients in the development of nursing for improving self-care through Orem/Underwood self-care model, the technique of interview still depends on each nurse’s own way of thinking. In this program, we will deepen our understanding of Orem/Underwood self-care model through lecture, case seminar and role play in three theme: Psychodynamic theory and Orem/Underwood self-care model; intervention technique in psychodynamic nursing interview for promoting self-care; and intervention technique for nurses to improve cohesiveness and function among the group of patients, their families, and nurses.

 

7. Dynamics and Intervention for Organizational Development

The theory and practice of the 0rganizational Development has been developed very much by the theory of psychoanalysis and Group Psychoanalysis in addition to behavior science. It also developed the executive coaching. My approach is reconstructed and integrated from Psychodynamic Psychotherapy and Group Psychoanalysis with the General Systems Theory. This program is focused on the organization of medical and helping professional institutions to make the psychotherapeutic wok effective. Psychodynamic psychotherapy especially for difficult patients with negative therapeutic reaction (NTR) often shake the institution where it is carried out or the whole environment of the institution could shake the psychotherapy work and bring quite destructive affect. Psychotherapists working in any kinds of organization need to do liaison work to protect their psychotherapy boundary in order to make it safe. Effective psychotherapy needs good enough psychological minded environment of the organization. You, psychodynamic psychotherapist, help curing patients in your psychotherapy work, and at the same time developing the organization good enough environment. We will learn basic theory of interactive dynamics of individual and organization, and method of analysis with intervention which gives it productive and creative way of use. Each participant should prepare for description of his/her own impasse in a given organization in which you are conducting therapy work or involving in any way.
